Restamping of B2 Visa on New Passport(lost passport)

Recently I had lost my Indian passport which had a valid B2 Visa( till Dec 2022). I dont have a copy of this visa.Currently Im on work permit here in Toronto.
I approached Toronto police to file a report and after all the documentation in Indian Consulate, I was able to get the new passport.
It has a reference of old PP number.
Can anybody tell me on how do I apply for restamping of B2 Visa? Do I need to fill the DS-160 again? What is the fees for it?
List of documents with me :
1)copy of lost Passport
2)New Passport(original)
3)Valid Work permit
5)Police report
6) If required, I can get Affadavit

Any input on this will be of great help. Awating responses.

Im working on replacement of Canadian visa in parallel.

You need to get a new visa and pay $160.
