Hi ,

In how many days a person can re enter US again after staying 4 - 5 months on visitor visa .
and after staying a month is India . thank you.

There is no rule around this. But to be safe one should at least spend the same time outside US as much as inside US on a visitor visa. To be extra safe double the time outside US than time spent in US.

what can be issues that could be faced at port of entry after spending a month in India if we return .

A much higher scrutiny, a much lesser I-94 and allowed stay. Worst and rare case deportation.
