DS 156 visitor visa B2

I applied for B2 extension, however extension got denied and I left US after the date in I94. It is obvious that I overstayed in US. As I read couple of threads, the visa my got void due to overstaying. I want to get another B2 visa for visiting US. What should I write in DS 156? Visa got cancelled or denied or revoked?? Please help.

If you over stay the Date on your i94 I don't think you can come back to the US so even if you apply for another visa your likely to be denied again. My opinion not legal advice.
