DS-156 - Spouse's Full name

I am filling ds-156 for my recently widowed father. What should be filled in Spouse's full name and DOB areas ? None or Late. name ?

Also, will they ask any questions about the spouse like when they died etc ?

Just write your mother's name when she was alive.
