What does 6 months per year mean?

Hello all,

I have heard often that when a person gets a B2 visa even for 10 years, they are permitted at the most 6 months. Does this mean:
1. 6 mths per visit?
2. 6 mths total stay per calendar year?
3. 6 mths total stay in the trailing 12 months?

Or does it mean something else?

Visa is valid for someone to travel to the USA. The duration of stay and/or admittance into the country is decided by the US Immigrarion officer at the U.S. port of entry. The maximum duration that can be permitted per visit is limited to 6 months.

Visa needed to enter into USA. But the length of stay is authorized by Immigration officer at the port. He/She will mention the return date on I-94 form.
