Hi,
My MIL's marital status is widowed. Do I have to still fill Spouse's name and DOB?
On Qn.18, it says Spouse's Full Name (Even if divorced or separated, include maiden name.)
On Qn.39, Was this application prepared by another person on your behalf? Since I am going to do this online form, Do I have to mention "YES" or is it safe to put "No".
On Qn.23, Can I give my email address?
Please clarify.
