i have fillerd the applcation of ds 156 for momhave few doubts

1) purpose of trip wrote

spend time with daughters family and tourism is it ok or write only tourism

2)

application prepared by whom asked , i did it but should i mention my name on behalf of mom, since mom is going to attend tamil interview and she is not good in english it will be foolish to write she did it..........so can i write my name there or is it some one with her now who explains her should fill it up so can i put my sisiters name who lives with her filled the application or..... kindly clear me.

2> You can mention your name or your sister name. It is more appropriate to mention your name.
