DS160 Form Query

Hi friends,
I am filling out form DS160 for my wife. I will be travelling on B1 visa from my company. She hopes to accompany me on tourist visa.

I have two questions:

1. There are 3 options to select at the beginning - 1. B1/B2 2. B1 3. B2
Please let me know which I need to select for her. (Option 1 or 3?)

2. Another page asks "Contact person or Organization in the United States". For either of them, we have to give the Street Address and Phone number. Since she doesn't know anybody, what should I fill in this page?
