Which Language to Pick for Doctors

Hello ImmiHelp,
My sister and brotherinlaw (age around 40) along with their children are going for VISA. They are both doctors (MD) and very well know English.. Should i take Telugu as the language or English ? But i was thinking if the English accent will be an issue.
If i take Telugu, will it be odd that doctors are taking appointment in Telugu, and cause some doubts in the interviewer's mind ? Please let me know.

If they are really uncomfortable with American accent, they can opt for telugu.
they will be interviewed by an american consular who is fluent in telugu as well.

Don't do that. Always stick with English. If they are doctors then they would be able to respond to the questions I n English very easily
