Visitor's visa for niece

Hi,

I am planning for applying visitor's visa for my niece who is 16 years old.
Just wanted to know the procedure and all the document needed.

Choose the country where applying.
Create an account .
Complete form DS160.
Pay the fees
take appointment for biometrics and interview.

Documents; school enrolment proof. Vacation proof. Funds proof (Parents' Tax return/salary slip/bank statement).
Invitation from host. Copy of host's visa.

Also one or both parents will have to accompany her to the interview even if they are not applying. If one parent goes then the other
should send an NOC .
