my actual name given in working place as padmavathi. my sons birth certificate have my name has padma. my passport calls name padmavathy as privious name and now it is padma. Now my problem is, I have interview this month end. NOC from office is under name padmavathi and also my propery is under name padmavathi. So at the time of interview, if they asks me 'why I have not got NOC under name padma', what I have to reply and what document i have to carry to interview. Please advise me

Let's say your name in passport is 'Padma' and your name at few other places is 'Padmavathi'.

DS-156 has couple of fields.

Name: Write your name as in the passport there. That is, 'Padma'.
Other Names Used: Padmavathi, Padmavathy

This will solve your problem.

This is a common problem in India. If someone's name is Suresh, he may also other names such as Sureshkumar, Sureshbhai, Sureshchandra, Sureshlal etc in various documents. And consulates know this and understand that.
