Doubts in DS-157, please help

Hi,
My father is a business man And in Q 17, list all educational institutions, is it required to give the telephone numbers of the instituitions where my father studied? Its been a long time sice he finished his education and we dont have the numbers. Its going to take a while to get all the numbers as they are in different cities, and I dont want to delay taking an appointment because of that.

Any help is highly appreciated.
