Once I get a visitors visa b2 for tourist and family visit. Can I take medical treatment there in that visa or have to re apply for visa on medical purpose?

No you do not have to apply again. Visa given is combined B1/B2 so you can use it for any purpose covered under B1 business or B2 tourism . Medical treatment also
comes under B2.
You just have to declare the correct purpose of your visit at US entry , so they can stamp your passport with either B1 or B2.
