ds 157 name filling

i have a surname and a given name in passport.so in ds 157 what should i write for last name.my given name is a small name like r****.and my surname is like two names like boris becker.so whats my first name and last name.
i think first name is r****.last name is borisbecker.full name mean borisbecker r****.is that correct.ds 156 they ask whats first and middle name.i think i can
only give answer r**** to that.

First Name = Everything in Given Name line
