    Hi,

    My mother is born in Karachi in 1937 (Before independence)
    She does not have any birth certificate, but while making passport, her affidavit was made that she is born in Karachi (Undivided India)
    While filling up D-156 form it asked for City,Country and State of Birth.
    In her passport it is mentioned Place of Birth as Karachi (Undivided India)
    So filled City = Karachi,
    Country = Pakistan (Since if we fill India, it will never give state of Sindh)
    State = Sindh
    In Nationality = India

    Also do we require any affidavits or her voter's ID or any other document while facing interview to prove her nationality?

      I believe you would put it down as being in India. This is naturally because at the time thats exactly where it was!
