Question about filling I-134 form

Hi,

I want to sponsor to my parents and grandmother and just confused how to fill all 3 names in I-134 form, since it just has fileds to fill the name of a person (Dad) and the spouce or children following that person. can some one tell me where i need to add the details of my grand mother.
Any help is really appreciated.

YOu can fill just one I134 too if they are going to the interview together. I did just one when my parents had their interview.
