Help filling out the DS-156

I would like to get a tourist visa for my mom. I have paid the application fee and VFS service charge at one of the designated branches of HDFC bank in Chennai and received the fee receipt number.

My mom has been visited USA in 1991 for a month on a tourist visa. She lost that passport. In the new passport, no visa details are present. We know the old passport number, the month of the tourist visa was issued and the month of she visited the United States.

Now, I am trying to get an interview appointment through www.vfs-usa.co.in website it requires me to fill out date, month and year [dd-mm-yyyy] of the most recent visit to USA in the form DS-156.

Question 1: Since we don’t know the exact date of the most recent visit to USA or the visa issued date. How do I fill out these questions in the Form DS-156?


Question 2: I will be preparing the DS-156 for my mom. Is it ok to put my name and address in USA on the column where it asks "Person who fill out the form on behalf of the applicaiton"?

For Q2, yes, u can put ur name and address.

For Q1:

She lost passport here? Evenif u don't know the exact date and time of her visit, u should know atleast the month? so just put approximate date with correct month and year.
