How to fill I-134 for parent and in-laws

Hi,
I need help on filling I-134 form for my parents and in-laws.

In-law's interview is scheduled on 22-Sept-2015
Parent's interview is scheduled on 23-Sept-2015

1. Do I need to submit 4 separate I-134 form for my father, mother, father-in-law and mother-in-law?

2. Since my In-laws are going first for the interview, do I need to mention my parents name (under section 9 - Previously submitted affidavits) in the in-law's I-134 affidavit form?

3. Please confirm if I must mention my In-law's name (under section 9 - Previously submitted affidavits) in the parent's I-134 affidavit form as my parents are going for interview next day after my in-law's interview.
