Naming protocol in I-134 form for B-2 visitor visa

I need to apply for B-2 visitor visa for my parents and have some questions. Kindly revert.

For example, the name is filled as below In Indian passport:

Surname: Blank
Given names: kritu kumar

1) I have submitted DS160 as below, I hope it is fine.

Surname: kritu kumar
Given names: FNU

2) How should I fill the names in I-134 form

Family name
First name
Middle name

3) When we create profile on below website for paying fees and taking appointment, should i follow the same protocol for First name/last name as i did in DS160
