what to fill in last name in ustraveldocs profile if sur name is blank in passport

Could some please tell me if surname is blank in passport what should i fill in first name while creating the profile at below website.

For example, if given name is Vishnu pal in passport and surname is blank should i create the profile as below

Last name: Vishnu pal
First name FNU as in DS160 form


Also, should i follow the same protocol in I-134 form also OR, i can leave the surname blank here
