I134 - Residing from Info

Hi ,
Need advice on filling the 2nd column "residing in the USA since (date)" in I134.
I came to US on H1 in 2012 Dec and returned back to India on Sep 2013.
Again, I came to US on same visa in March 2014 and I have been here since then .

Should I mention Dec 2012 or March 2014 in the 2nd column ?

Note: Bank account verification letter will have account start date as Dec 2012 ..

That question is for the petitioner. If you are the petitioner and sponsor, then you should use the last entry date as the response to that
question.
