for blocking appointment date

My parents have paid the HDFC fees and got the receipt. Heard that it would get activated in 2 days. I"ll be booking the date online from here in US on their behalf. Now, after this, in order to book the date for them what all details are required for me to book an appointment for them?
Should I ask them to send me the details in D156 and D157 form alone?
will I be required to fill some other form for them apart from D156 and D157 while booking the date online?

You would need the HDFC receipt numbers. You need to fill out only Ds165 and 157 when booking the appointment online.
