Really need your help with DS-160

Hello, I am kindly asking for your help, got stuck filling out DS - 160

1. I would like to apply for US visa to travel with my boyfriend(he already holds a US visa) so, should I pick 'Yes' in 'Persons traveling with you' section and mention him?

2. There is a section 'Address Where You Will Stay in the U.S.' we are planning to travel around the US (couple of states) so my question - is it enough to specify location only of one hotel? Because as far as he told me - there should be some question about "specific travel plans' but I am not getting it..

3. Regarding 'Contact Person or Organization in the United States' will it be OK to mention my friend? she is US citizen, but we are not going to see her during our visit

And a last question - please advice on type of visa..I am tourism oriented only, so should I pick B2 only or B1/B2 ?

hes non US resident, so I guess physical address of at least one hotel that we are going to stay should be enough..
