B2 extension for parents

I would like to file a extension for B2 Visa gor my parents. I woulf like to know if I need to fill two different applications for my mom and dad or fill just one and add the other in the supplement form.

Also, how about the processing fees, do I need to pay $290 + $85 for each person or both together?
