She had been approved for an internship at a German school with the help of a (female) German friend, but chose the US internship over the German one. Now it's too late to apply for a visa to go to the other one. I don't know what impact this will have on her ability to graduate, I'm given to understand it was a required part of her last year.

Known her for two years. Of course I care about her. I hate to think I got her hopes up just to ram her into a brick wall, moreso if the rejection was my fault.

After she becomes a doctor and with stable job she can get tourist visa. For the moment it's very unlikely but this is not the end of the world. It won't ruin her life or any professional chance for her. If you have feelings for her, you can apply for fiance visa if you both decide to be together. You are making a huge deal. Your invitation might not have helped her, but with her nationality and the amount of time of her internship they would not grant her visa probably anyway. You are beating yourself up unfairly.
