3. DS-156 – non-immigrant visa application form
4. DS-157 – supplemental non-immigrant visa application form
5. HDFC application fees Receipt
6. DD for 2,300 in favor of ‘American Consulate General, Chennai’
7. Interview Appointment Letter
8. Letter to the Consulate of why are they are visiting USA
9. Bank Statements, Bank Balances & Tax Returns for the last 3
years - Original
10. Photocopy of First Page, Address Page & the Last Page from the
current passport
11. Business Card
12. Family Photographs
13. Letter – Nature of Visit to USA
14. Affidavit letter
15. Proof of Property – Indicate Strong ties with the Home Country
16. Company Registration Certificate - Original
