getting visiting visa for 2 weeks

Dear Sir/ and Madam,

i wish to go to u.s.a for 2 weeks with my 2 kids under 16 years. my husband is not going to joint with us because he will be in out of the countey at the moment and he is bussy.i got a invitation letter from my usa friend. and my sister in law going to sponsar me (financially) during our stay in usa.
what we need other than these?
from my sister in law is there anything need affidavit?
is there any special affidavit forms 1-134 affidavit of support
