VO took passport but DS160 status went from issued to refused?

VO took passport but DS160 status went from issued to refused?

I was checking the status and it says "ISSUED" and then the next day it went to "REFUSED" and update date on the status seems way off , says April when it should be May i.e yesterday or today?.
