urgent info needed

iam sending all the necessary documents for my father for his appointment on apr5th.
while filling d 156, under the question"who is accompnaying u in the journey" i had given my mother's name(she has valid visa already).
while filling i 134 i did not show her name as advised here in the forum
will it be a problem?

i tried editing in the d156 form, it is deleting my mother's name, but inthe non editable ques 2nd, it asks for " residing state"(which automatically has goneand it is not allowing to choose it but keeps asking for it when i try to edit. what to do about it?

one more question is we got tax return papers and w2s for 2005 and 2004, but for 2003 we have only w2 in hand..what to do about it?
urgent advice will be greatly appreciated.

I think thats ok. (I would have included her name in I134 if I were u, though). You can always tell that, since she already has the VISA, u didn't include her in the affidavit. Also u can take a new printout of DS 156 and fill all details so that it matches I134. Take this new printout and give it if they ask.

Q2. W2 should be fine
