d156 doubt

when i had filled up d156 form online, i had not typed my father's little different surname under "other surnames".
so i tooka print out of d156 form without barcode, filled this info along with all prefilled info.
my doubt is... has my father to submit both d156 forms in the beginning or has he to show the second one only if the matter arises( the other surname is in my father's pension book)
please advise,

They will ask for each form (like !34, Ds 156 etc..). Give both of DS156 (online and manual) together. In the manual copy, write the change (in ur case, the change is the 'other surname') with red ink so that they can easily identify what the change is..
