Canada Visitor Visa From USA

Hi,

I am planning to take my mom, who is visiting us from India, to Canada this September. I checked online to apply for visitor visa for her.
I found the PDF with the following name.

MM 5257E Schedule 1 : Application for a Temporary Resident Visa Made Outside Canada

Please confirm if I am using right application.

That sounds correct application for Canada visa. You need to go to the nearest Canadian Consulate and apply in person.

This application requires only Name and Date of birth of the applicant. Rests of them declarations about military etc.
There is no mention of passport, nationality etc. I guess something is wrong. Can any help.
