Application form DS-160 for Visit Visa is an Individual application form?

Hello
I have submitted DS-160 for my mother in law and his two sons.
While filling application online they asked question who will be travelling with you. In that section i entered her both son's name.
Now my question is that do i have to fill separate application for the two of the Son, or one application is fine with the son's name on it.

Also after filing out application they never asked for fee. How can i pay application fee online??

Your response can help me taking immidiate action.
