I-134 Affidavit Of Support

Hello,

Have couple of questions in filling out this form (I-134 Affidavit Of Support): This is for my parents who are planning to get a Tourist Visa.

1. Should I sign this document in front of a Notary public agent or its not needed and signing it myself will do ?

2. Page 1, Part 2, Sections 12a - 17 --> My sister (34 years) will join my parents for this trip. But she might apply next month once she gets her NOC as she is a government employee.. Should I mention her or leave it blank in 'Beneficiary's Children' ?
3. Page 3, Part 3, Section 3 --> I just mentioned my salary. Should it just be my salary or entire household salary as even my wife works..
4. Page 3, Part 3, Section 8b --> I mentioned my car loan balance and both me and my wife share it for monthly payment.. Is it ok to mention ?
5. Page 3, Part 3, Section 10a - 21 --> Since my wife is working I did not mention any Dependent or should I mention my wife as Dependent and mark as 'Partially Dependent on my for Support'

1) No notary required. You can sign by yourself.
2) Don't write your sister's name under beneficiary's children. She is not a minor. She will require a separate Form I134 if you are sponsoring her USA visit as well.
3) I think it is better if you fill out two separate Form I134, a) where you are the sponsor listing your details such as salary and b) where your wife is the sponsor listing her details such as her salary. Edit: I am actually not sure if two people can sponsor one person. Since one person can sponsor many, I assumed many can sponsor one person.
4) I believe that space is for house owners who have mortgage left. I personally did not write my car monthly payment.
5) That's correct. You should check partially dependent.
