for Q37 - you should mention anyone other than the sponsorer. For eg. in my case I was the sponsor and my sisters are here. So one sister I had to mark as "daughter" and permanent resident in the space provided. I could not list for other sister since they provide only one column for daughter/son (may be I got away !) but my parents did mention clearly about me and my siblings being in US.

for Q39 - I did prepare the initial version but there were changes my dad had to make so he edited the document so we marked "No" and as far the visitors (you're sponsoring) know what's in this form, you can mark as No but if the don't then please mark it as Yes and ...

for Q40 - depends on Q39.
