Apply from Kuwait

Hello there,

I'm planning to apply for USA visit visa (B2) first time from Kuwait, I would like to have expert opinion in few things below,

1. Property documents need to translate & valuation also or just photocopy will be fine, those documents are not in English.

2. Recently had few changes in my passport, such a Date of Birth & Minor change in name, which is already documented in Kuwait documents but on my previous passport have some travel history, such as Schengen visa which is no more valid though, but wanted to show the travel history on DS160 form, now my question is,
Embassy Name correction paper (Where is mentioned all the changes) will be sufficient or do I need to make Self declaration or Affidavit against those changes,

Embassy Name Correction Paper format is like that in below,

"TO WHOM IT MAY CONCERN"

"Embassy of the ............Certifies that my name..., Father's name..., Mother's name... passport No.... Issued Date & Palce.... upto Valid date....

According to the passport his date of birth is.....

All Concerned are requested to do the needful in his favour"
