Statement Letter from the Bank

Hello All,
I went to my bank (JP Morgan Chase, formerly Bank One) to get the letter stating my current balance, average balance last year etc.
I was told that the branches do not issue these letters anymore (they cited it was because of the patriot act) and asked me to fax a request which I did. The bank just sent me the document which is not exactly how we need it:
It states "immigration request" on top which is not what I had requested. It's not addressed to anyone, and does not have a total amount deposited last year.
I'm really frustrated and was wondering if anyone has encountered the same problem.

Any comments/inputs are appreciated.

Bank Denies letter

The Bank (JP Morgan Chase) refuses to give anything other than the total present balance and the average balance for last year.
It's a faxed copy with no structural letter format.
They keep saying after the patriot act things have changed and they cannot issue a letter anymore and this is all they can provide.
I really don't know what I can do.

Any help is appreciated.

Chase bank statement

Hi,
I got the same document from the bank with the title 'Immigration request' on top. That looks quite ominous, especially since we don't want our visiting parents to appear as immigrants.
I've sent them the letter ( which looks like a photocopy ).
Did your parents already go for the interview? Did the document pass the test?
