Parents Tourist Visa - Name and Address Questions

Hello All,

I am on H1b visa which is valid till Oct 2018. I am applying for my parents visitor visa, have a few questions regarding name and address.

Dads Passport has his middle name and a word in the address in spelled Gobind. His passport has Mom's full name.

Moms Passport does not have dad's middle name in spouse. Only First and Last. The word from address is spelled as Govind in her passport.

How do I go about filling their DS160 and i134? Do I specify dads name without middle name in other names used?

Use each person's name as per their own passport. Don't take names from 'Spouse Name'.
