Help on I-134 (Items 7 & 9)

I need help on the following two questions while filling I-134 - Affidavit of Support:
1. Item 7: Deposit in savings banks
I have entered the total sum of savings in all my bank accounts. Do I need to submit a verification letter from EACH bank? If so, can I just enter the savings from one bank since it is sufficient for the purpose of sponsorship? What supporting evidence is sufficient for stocks?
2. Item 9: Previous affidavits
I had submitted an affidavit for my wife when she entered the country on H4 visa. Should I mention this? I know instructions on this site state that one should NOT mention visa petition for spouse in Q10, but what about Q9?

1. If you are showing assets from one bank, you just require paperwork from one bank. Otherwise, you require from each financial instistution.
