form d156 very urgent

I have a question to ask..
My interview is comming up very shortly..
The form d 156 has to be printed back to back..
but the supplement comes on the extra page..
is it ok???????
please need advice urgently

Based on the questions #29,30,31 in DS 156, some applicants will get a supplemental sheet while doing online appointment. If you have this, it has to be attached with the back to back page of the DS 156.
