Very Urgent ! Name Interchange

Hi,
I have strange situation

In my mother's Passport Name and Surname have been interchanged.
But In Surname field in Passport the Sur is been scored out
And in Given Name Given is been scored out.

This is the second attempt for them.In first attempt we had
written the Actual name and surname.
This time also write the Actual name and surname and if asked
tell visa ofiicers that as Sur is scored out so that should be considered
as Name only.

Any help is really appreciated.

Can you please give an example of the names? I am not able to make what exactly is going on. You don't have to use real names. Just some names to give me some idea.
