I have read a lot of conflicting information on the internet. It seems Ukraine has a 30% refusal rate for Visas. What is required to guarantee a successful Ukraine Visa application?

Regarding salaries and situation in Ukraine 30 per cent of refusal is not so high.
But as they often don't look at documents and make their mind after your answers it might be difficult.
