URGENT: form I-134 question

HI

I am sponsoring my inlaws and immihelp website says one form I-134 for each, Does that mean same copy twice or Do the name on question 3 "This affidavit is executed on behalf of the following person" should be changed vice-versa. i.e On one form my father-in-law information and accompanying person mother-in-law info On the second vice-versa.

The appointment is in june 2nd week. Please reply.

U don't really need 2. Just one form is enough.
