US tourist Visa for child with Divorced Parent.

I am looking to get by neice to US - who will travel With her grand parents i.e. my parents. Her Mom is Divorced and has custody of teh child as per teh Divorce agreement. Would be great if some one could help clarify a few things regardign teh B2 visa application for her:

1) While filling the DS160 do I have to mention Fathers Name/Details or can I leave it as Do Not Know?

2) The Mother of the child also does not have a US Visa, who needs to accompany the child for teh visa interview Mom I assume ?

3) Is there any other complication/things to know in this case?

1) While filling the DS160 do I have to mention Fathers Name/Details or can I leave it as Do Not Know?

The kid does have a father's name in passport, hence it should be mentioned in the DS160 as well.

Kids under 14 yrs of age really do not need to attend the interview - you can also use the dropbox facility.

3) Is there any other complication/things to know in this case?

You'll need father's permission to take the kid out of the country, the same affidavit will be needed at POE in USA.
