I am applying for B1/B2 renewal. However, my last visa that was issued was in a different category (H4). Now, while filling out the DS-160 for B1/B2 renewal, how should I answer the question- date last visa was issued? Should I provide the details of my old B1/B2 visa or H4 visa?

Since you are filing for B1/B2 renewal, you need to file B1/B2 visa issuance date.

i guess you may have to give the last visa details irrespective of the type of visa since the next question in the page asks 'Are you applying for the same type of visa?'
