For 2 sponsors do I need to prepare 2 I-134's as well ?

Both myself and my wife, are sponsoring my parents (both mom and dad), and we had given both our names as sponsors in the DS156 form.

For the I-134, do we prepare two separate forms in each of our names (as per the guidelines on this site, then we need to have 4 I-134's in total since each sponsor needs to have one form for the person they're sponsoring) or do I need to prepare just 2 I-134's with my name on them as the sponsor, but including my wife's income as well in the financial data ?

If my wife's name is also listed as a sponsor on the DS156 form, where do I put her name on the I-134 document ?

Also, as an added question, is the I-134 something that the VO looks at or takes always, or is this something that they may not even look at in many cases ?

go for 2 I-134 forms one for dad and another for Mom

Put your combined income in both forms (as you both planning to sponsor)
and make sure that you provide related financial docs for this from both of you, such as pay-stubs, W2 and tax returns, emp letter etc

Most of the time VO will definately look at this doc

Thanks for your quick reply. Right now, I prepared the form with financial data showing our combined income (mine and wife's) but in this case where do I put my wife's name in the document ? Do I just put my name but include her salary as well or do I need to add her name on the document to maintain that we are joint sponsors ?

I dont think you can put both names, as there is no provision for two names on this form.
While specifying dependants col#8, say partially dependant, and ask your parents to tell VO that they are sponsored by both of you and to show docs from both of you when asked
