    Hello,
    I have applied for a B2 visa at the Mumbai Consulate and have an appointment for July 3rd. I have filled the DS 156 online and have submitted copies at the VFS office.
    I have a query about the DS 156. As per my passport my surname row is struck out and my given names row says "Shivaraj Natarajan". I had called up the helpline at Mumbai asked about filling this in the DS156 before submitting the same.I was instructed to fill in Item No6: "Surnames" as Shivaraj Natarajan and Item No 7: first and middle names as FNU. I had called thrice to confirm this and I received the same reply.

    While surfing some internet forums I have come across different interpretations to this. I wish to know if this is filled in correctly and if not so then whether I can correct it at this stage since my appointment is only 5 days away.I hope my application is not rejected due to any error in this as I have acted as per instructions of the VFS staff.

    FNU stands for First Name Unknown. Did you call the consulate aswell, apart from the VFS. On my passport and i have only one name (due to passport employee err) but on my GC it reads FNU & my name.

    Whenever i'm at security at airports, i get teased, oh this is like Madonna or Prince !

      what they said is correct. U have to put FNU and enter ur given name in the surname field. For B2 visas its not a problem.

        I have applied for a Tourist Visa for my mother last day and the Interview date is on May 02.
        Her name in Passport is like this:

        Surname: ....................(blank)
        Given Names: MARY VARGHESE

        On the Online Application Form I entered,

        Surname: MARY VARGHESE
        First and Middle Names: FNU i.e., First Name Unknown (On website they asked to use like that if I want to)

        Is it correct or not?
        In my Interview Letter the name displays as FNU MARY VARGHESE

        Whether it make any issues later when I apply an Immigrant Visa or GC for my mom?

        If it is wrong please do I need to cancel the Appointment since I am not able to modify those fields?

        Is there any problem If I split her name and write first name in first & middle name cloumn and last name in surname column in DS-156?
