  • Name difference in Passport and Visa

    My mother got her visitor visa, but despite using the "FNU" method in the application, the issued visa has her name written differently.

    In the passport:
    Surname: <blank>
    Given Names: SEEMA AHMAD

    In the visa:
    Surname: AHMAD
    Given Name: SEEMA

    Can anyone advise how the name should be entered on the customs declaration and I-94 form prior to arrival in US? Thanks.

    Just enter according to visa.

      I appreciate your advice. However, can you please explain if it is based on an official rule or any real experience?

      It appears to me that the "Surname" is a label only used in the passport. I have obtained a copy of the Customs Declaration form and the full I-94 form. Both of them require a "Family" name and "First (Given)" name.
