Urgent DS-156 question

Hi

We have prepared DS-156 for my mother visa interview.. She does not know english and she will appear for interview in Gujarati language..

Do we need to completer Q.40 of DS-156? This is about who prepared the application. since my mother does not know english i think i should complete this question.... please reply urgent so i can change mother application online..
