I-134 question

I am preparing documents for my Mother's visa. She has been here twice for 3-4 months. In question 9 of I-134, you are required to provide names of people who you have submitted I-134 in past. I have 2 questions:

- there is space for only 2 rows. If you have more than 2, do you just write below?

- do I have to include entry for my Mother's affidavit submitted before apart from others?
