Question regarding photo on DS-156

Do we need to stick or staple photo on DS-156.
Also does the photo need to be signed ?

Please clarify. Somehow I can not access the DS-156 on VFS web site and my parents asked me this question.

Stick.... (make sure that you follow the stds for photo and paste just it the space provided).
No need to sign.
